#4 cylinder sucking in coolant! But from where please help!!!

I have a ka and my problem was i started getting coolant into my cylinders alot in # 4 and when i took everything apart i got the head completely redone. The head was warpped .01 of an inch so the machine shop resurfaced it. I replaced the head gasket and bought new head bolts. When i put the head gasket on i used the 5 step torquing process. It was (1) 22 ft lbs (2) 55 ft lbs (3) break bolts loose (4) 22ft lbs (5) 55 to 62 ft lbs. The final torque i torqued the bolts to 60 ft lbs. I got everything back together all 4 cylinders have great compression all of them are at 160. But when i put everything back together im still having the same problem.

When i pulled the head off i noticed only # 4 was getting wet this time and i looked at the gasket and the was a small knick at the coolant passage. I had the head redone at napa. The one thing that is really confusing me is my intake manifold is filling up with coolant as well. Can some body please help this is the 3rd time ive ripped apart his motor
