Mostly it's framing the subject, determining where you want your focus, and if it's a long-exposure shot (like the blurry one in ritz's post or any night picture where everything is clear), determining how long an exposure you need.
Essentially, I just fuck around with my camera until I get the pic I want.
I've never taken any photography courses (I wish I had, and may yet), so everything I've done is just based on me experimenting. Most digicams nowadays can take a nice shot if you frame the subject right and focus on what you want clear.
When you're going for a lack of graininess in your images, big megapixels and large lenses come into play. However, for 90% of folks out there, it can be a bit grainy and still look good.
